Can people do my questionnaire?

To be honest, i'm not entirely positive that i've worded things correctly, or made it as simple as i could have, but i've tried! I may have to redo it if people just look at it and think 'oh f*!? that', but to be honest, it's not a questionnaire i COULD have done multiple choice, i need in depth answers... SORRY!
